FIPPA Consent Form

Pursuant to Sections 33.1(1)(b) and 30.1(a) of the British Columbia Freedom of Information and Protection of Privacy Act (RSBC 1996, c. 165), I, <Student Name> ("Student"), give my consent to Simon Fraser University ("School"), to disclose and store outside Canada my personal information under the terms and conditions noted below.

  1. The personal information to be disclosed and stored outside Canada is: Student name, nickname, Kadenze account password, Kadenze user ID, home or business address, email address, telephone number, credit/debit card information (such as the credit card number or billing address), social security number (students residing in the United States of America), bio, avatar/user image, gender, date of birth, location, time zone, languages spoken, educational background (such as school, major, education level, and status), employment background (such as status, industry, and employer), social media links, personal websites, and other projects or media added by the Student to their user profile / portfolio, Submitted Student work, and grades assigned for submitted work as related to the program Generative Art and Computational Creativity ("Program"). Posts, replies, messages, and other types of communication made by the Student on any course forum or gallery related to the Program, and in direct messages with other users
  2. My personal information may be disclosed and stored outside Canada to the following person(s) and jurisdiction(s): Kadenze Inc. ("Company"), located in the United States of America, and any personnel employed by Company
  3. My personal information is disclosed and stored outside Canada for the following purpose(s): To provide access to learning material for the Program and any of the Program’s courses, to enable Student to participate fully in Program activities such as submitting coursework, participating in Program course forums and student gallery, to evaluate Student’s learning progress in the Program’s courses, and to assess Student’s level of competency in the Program courses to assign course and Program grades
  4. This consent to disclose and store my personal information outside Canada expires within 1-week ( 7 days ) of my request to delete my account

No disclosure, storage or use of personal information about me beyond the terms described above is permitted without my express written authorization or unless permitted by law
